Fee Schedule
- Cash based practice
- Set rate of $140 per hour
- Checks/Cash accepted
- Invoice for insurance submission provided - patients are responsible
for submitting all claims.
- Devine Intervention Physical Therapy cannot treat Medicare part
B patients. This is due to the fact that Devine Intervention PT
does not have handicap accessible entrances and exits which prohibits
DevinePT to apply for even non-participating status. Therefore,
we are not able to treat Medicare Part B patients even if they
were to pay cash and submit the claim themselves.
